This request is for a Public Safety Officer to be present for a campus event. Staffing costs are charged back to the department/event at the rate of $35.00 hr. with a 4-hour minimal.

This form will be used to appeal parking tickets

If you plan to be away for an extended period of time and wish to leave your car in one of the Seminary parking areas, please follow these guidelines:

CRW residents– park in the appropriate CRW parking lot or the lower level of park deck.
Employees & Main Campus residents– park either in the lower level of the park deck or in the Library lot

Repair request creation

This form is utilized by Faculty Members when it is time to update and replace their computer in accordance with our Equipment replacement cycle.

This request is to have a physical key made for a particular room on campus. This request will require the approval of the Director of Public Safety.

Use this to make a request of Facilities

PTS Alumni must present an alumni card to access library resources and enter designated areas on campus, including the parking structure. To begin the process, use the link below to request your alumni card. Keep in mind that you’ll need your PTS network login to access the form.

This service request can be used if you or a dependent have lost a PTS ID card. This request can be used for both Students and Employees. This request must be completed by either the student or the employee.
